We’re physician-led and whole-person: medical care that is integrated with practical coaching (meal planning, label reading, budgeting), weekly fitness support, stress/sleep tools, and culturally tailored education. We prioritize access with a simple application, bilingual services, and low- or no-cost enrollment supported by grants and community donations.
Adults with insulin resistance, prediabetes or type 2 diabetes—especially uninsured or underinsured community members. If you’re unsure, apply and our team will review your eligibility.
Thanks to grants and donors, many participants qualify for free or sliding-scale enrollment. We’ll confirm your cost (if any) during intake and never enroll you in services you can’t afford.
Physician visits, medication review/adjustments, nutrition education, grocery guidance, fitness coaching, goal setting, lab tracking (and CGM when appropriate), plus ongoing check-ins and support groups.
You’ll meet with a clinician for a medical history, medication review, vitals, and baseline labs; set personal goals; and receive an initial nutrition and activity plan.
Most patients have physician visits biweekly or monthly, with weekly coaching touchpoints (nutrition/fitness), book club, and/or support group sessions.
Absolutely. We’ll coordinate with your PCP or serve as your medical home if you don’t have one.
A photo ID, current medications/supplements, and recent labs or glucose logs.
Most patients complete a 6 month core phase followed by a maintenance plan with less frequent check-ins.

Many patients see improvements in A1c, weight, energy, and blood pressure within the first few months. Results vary by individual effort, medical needs, and consistency—we’ll tailor support to you.
We follow HIPAA standards and maintain strict confidentiality for all medical records.
