We prioritize access with a simple application, bilingual services, and low- or no-cost enrollment supported by grants and community donations.

Who is eligible to enroll?

Adults with insulin resistance, prediabetes or type 2 diabetes—especially uninsured or underinsured community members. If you’re unsure, apply and our team will review your eligibility.

Thanks to grants and donors, many participants qualify for free or sliding-scale enrollment. We’ll confirm your cost (if any) during intake and never enroll you in services you can’t afford.
